All students living on campus and all student-athletes (living on or off campus) are required to submit health paperwork each year. We use a system called Healthy Roster for our health forms.

Health Forms Required for Students Living on Campus

The following forms must be completed before a student can move into their residence hall or campus house.

  • Midway University Statement of Authorization (Fillable on Healthy Roster)
  • Midway University Student Health Form (Fillable on Healthy Roster)
  • Resident MenHep B Form (Fillable on Healthy Roster)
  • Copy of Immunization Records - This document must be in English.​ (Upload into Healthy Roster)
  • Student-Athletes have additional required forms (review athlete forms requirements)

Healthy Roster Login

 

Access to Healthy Roster is granted beginning June 1 and once you have paid your tuition deposit. You will receive an email to your Midway email with an access code from Healthy Roster that will allow you to create an account and access the forms. This email often goes to junk mail, so please check there regularly. The instructions below will guide you through the process of finding the fillable forms and how to upload documents. If you have paid your tuition deposit and you still don't see this email, athletes should contact at@midway.edu and students not planning to participate in athletics should contact studentaffairs@midway.edu.

 

Healthy Roster Instructions

  1. Login into Healthy Roster.
  2. Once logged into Healthy Roster, click on your name under 'Athletes' (NOTE: the system was set up for athletics, but is used for all students).
  3. On the page that says 'Athlete Details' select the "Documents" tab. If on Phone: click the '+' OR if on Computer: 'Upload a New Document'
  4. Click the "Select Document Type" then select the one of the following forms that are fillable forms, complete it fully, and click the submit button. Be sure to complete each required form. (If on phone click 'Select File' to view form.)
    1. Midway University Statement of Authorization 
    2. Midway University Student Health Form 
    3. Resident MenHep B Form 
  5. To upload a copy of your immunization records, under "Select Document Type" choose Copy of Immunization Records. You can attached a .pdf file or use your phone to scan document with camera and upload.
